Description
During WP-004 (concrete cover survey) at chainage 0+120 east slab edge, minimum cover measured at 38mm against the 50mm spec (drawing C-301 rev D · note 4.2). Reading taken at 6 points over 3m length. Average 42mm; minimum 38mm at point 4.
Root cause
Bar chair spacing exceeded specification on east edge run. Cause: bar chair stock substituted mid-pour (Bayswater confirmed). Spec failure not caught at HP-006 (footing release) due to camera angle obstructing east edge.
Corrective action
Bar chair stock returned to spec for remaining pours. Pre-pour edge survey added to ITP-CONC-01 HP-006 procedure. Updated procedure issued to Bayswater 14 Aug.
Proposed disposition
Accept as-is · with engineer's certification. Coring sample taken at point 4 · 7-day strength + cover verified to 42mm minimum. Designer (BG&E) issued certification 16 Aug stating slab capacity not compromised. Cost impact: $1,200 (testing). No programme impact.
